Facts checked on 30 October 2026.
No semaglutide or tirzepatide brand sold in India is approved for PCOS (polycystic ovary syndrome); the Indian approvals and the US labels name weight management or type 2 diabetes1,2,3,4,5,6,7. Semaglutide is sold here for weight as Wegovy and Poviztra; generic brands, sold since 20 March 2026, are mostly for type 2 diabetes8,9,10,11. Tirzepatide is sold as Mounjaro and Yurpeak12,13. In all six small PCOS studies, average weight fell14,15,16,17,18,19. Semaglutide was tested at 0.25 to 1 mg a week in groups of 20 to 100 women15,16. Tirzepatide was tested at 5 mg in one trial of 60 women, and liraglutide in trials of 72 and 82 women17,18,19. The US labels say semaglutide is stopped at least 2 months before a planned pregnancy, and that tirzepatide may make the contraceptive pill work less well4,6. Whether any of them fits you is a doctor's decision.
PCOS and each GLP-1 medicine sold in India: status on 30 October 2026, from US labels, the NHS (UK), CDSCO minutes, Indian reports and PubMed abstracts1,2,4,14. Semaglutide and tirzepatide approvals are for weight or type 2 diabetes, not PCOS. Not an offer or a recommendation. Facts checked on 30 October 2026.

How common is PCOS in India, and how often does it come with obesity?
A national study found PCOS in 7.2 % to 19.6 % of Indian women aged 18 to 4032. That study enrolled 8,993 women across five zones of India from 2018 to 202232. It used three sets of diagnostic criteria, so it gives three numbers: 7.2 % by NIH 1990 (US National Institutes of Health), 13.6 % by AE-PCOS (Androgen Excess and PCOS Society) and 19.6 % by Rotterdam 200332. The criteria combine features such as hyperandrogenism (raised male-type hormones), oligomenorrhoea (infrequent periods) and polycystic ovaries32.
Among the 1,224 women with PCOS, 43.2 % had obesity, 32.9 % had fatty liver disease and 24.9 % had metabolic syndrome, a cluster of signs such as a large waist and raised blood sugar32,34. Impaired glucose tolerance affected 9.1 % and diabetes 3.3 %32. The lead author reported grants from the Indian Council of Medical Research (ICMR); another author reported honoraria from Novo Nordisk, USV and Sun Pharma outside the work32.
The Indian government defines obesity as a body mass index (BMI) of 25 or more, and overweight as 23.00 to 24.9935. An Indian consensus statement says the limits of normal BMI are lower in Asian Indians than in white Caucasians36.
Is Mounjaro or semaglutide approved for PCOS in India?
No. Neither medicine lists PCOS among its approved uses in India or in the US1,2,4,5,6. In India, the news agency PTI reported that Mounjaro was launched for weight management by BMI and for type 2 diabetes1. CDSCO, India's drug regulator, records Wegovy as approved for chronic weight management in adults2. The brands and makers are set out in our pages on semaglutide in India and Mounjaro in India.
The Indian weight indications name a BMI and a weight-related condition; whether PCOS counts is the doctor's decision1,9. CDSCO's advisory of 10 March 2026 prohibits promotion that could encourage off-label use, meaning use outside the approved indication37.
What have the PCOS studies of GLP-1 medicines actually tested?
The six studies below tested 20 to 100 women for a few months; none was run in India15,16. In a randomised trial, chance decides who gets which treatment38. In a double-blind trial, neither the women nor the researchers know who gets what; in an open-label trial, both know33,39.
PCOS studies of GLP-1 medicines, from their PubMed abstracts14,15,16,18. Results are averages for the groups studied, not a forecast for one person. Facts checked on 30 October 2026.
| Study | Medicine and dose | Design and size | Weight result | Periods or pregnancy | Caveat |
|---|---|---|---|---|---|
| Carmina and Longo 2023, Italy | Semaglutide 0.5 mg a week | One group, 27 women, three months; six months in responders | −7.6 kg at three months; −11.5 kg at six months in responders | 80 % of responders normalised their cycles | No comparison group14 |
| Chen 2025, China | Semaglutide 1 mg a week with metformin, against metformin alone | Randomised, open-label; 100 women, 80 finished; 16 weeks | −6.09 kg against −2.25 kg | More cycle recovery; natural pregnancy 35 % against 15 % in weeks 16 to 40, after semaglutide stopped, all on metformin alone | Open-label; same registration and university as Yang 202615,17 |
| Bolek 2026, Slovakia | Semaglutide 0.25 to 0.5 mg a week with metformin | Pilot, one group; 20 women with prediabetes; six months | Mean 98.4 to 85.5 kg at five months | 60 % became pregnant | Pilot, no comparison group16 |
| Yang 2026, China | Tirzepatide 5 mg a week with metformin, against metformin alone | Randomised, open-label; 60 women; 16 weeks | −10.4 kg against −1.7 kg | More cycle recovery; more pregnancies in weeks 25 to 48, after tirzepatide stopped, all on metformin alone | Open-label; same registration and university as Chen 202515,17 |
| Elkind-Hirsch 2022, US | Liraglutide 3 mg a day against placebo | Randomised, double-blind; 82 women; 32 weeks | −5.7 % against −1.4 % | Free androgen index (a hormone measure) fell | Digestive side effects 58.2 % against 18.5 %18 |
| Nylander 2017, Denmark | Liraglutide 1.8 mg a day against placebo | Randomised, double-blind; 72 women; 26 weeks | 5.2 kg more than placebo | Bleeding ratio (a bleeding-pattern measure) improved more than on placebo | Nausea and constipation more common19 |
A meta-analysis pools data from different studies on the same subject40. One from 2024 pooled 176 women from four placebo-controlled trials; waist size, triglycerides and testosterone fell41. A 2024 review for the 2023 international PCOS guideline counted 11 trials of anti-obesity medicines and said published data are very limited42. A 2026 meta-analysis of 11 trials found a BMI fall of 1.38 kg/m², with low certainty43.
The large weight trials were not PCOS trials. STEP 1 (semaglutide 2.4 mg) gave −14.9 % at 68 weeks and SURMOUNT-1 (tirzepatide) up to −20.9 % at 72 weeks, in adults with obesity44,45. Novo Nordisk funded STEP 1 and Eli Lilly supported SURMOUNT-144,45. The PCOS studies used semaglutide at 0.25 to 1 mg, below the 2.4 mg weight dose, and tirzepatide at 5 mg, the low end of a scale with pens up to 15 mg4,15,16,17,46.
The weight schedules come from the US labels: semaglutide steps up from 0.25 mg a week to 2.4 mg, and tirzepatide from 2.5 mg in 2.5 mg steps4,5. In India, the Wegovy FlexTouch and Poviztra pens carry 0.25 to 2.4 mg, and the Mounjaro and Yurpeak KwikPens 2.5 to 15 mg46,47,48,49.
Which GLP-1 is best for PCOS?
None of the PCOS studies above compared one GLP-1 medicine with another, so they cannot say which is best14,15,16,17,18,19. The one head-to-head trial, SURMOUNT-5, gave 20.2 % average weight loss on tirzepatide against 13.7 % on semaglutide at 72 weeks50. It enrolled 751 adults with obesity and no diabetes, not women with PCOS50. Eli Lilly, the maker of tirzepatide, funded it, and it was open-label50,51.
In India, the indication differs by brand: Mounjaro and Yurpeak for weight and type 2 diabetes, and Ozempic, PharmEasy says, for type 2 diabetes only1,3,13.
Can semaglutide affect your period?
Semaglutide is not approved as a treatment for periods, but small studies report cycle changes alongside weight loss4,14. In the Italian study of 27 women on 0.5 mg a week, 80 % of those who responded had normal cycles by six months, with no comparison group14. The Chinese trial of 100 women reported more cycle recovery on semaglutide 1 mg with metformin than on metformin alone15. The tirzepatide trial of 60 women at 5 mg with metformin reported the same direction17. In the Danish trial of 72 women on liraglutide 1.8 mg, the bleeding ratio improved more than on placebo19. The 2026 meta-analysis of 11 trials found the evidence insufficient to draw conclusions on menstrual regularity43.
Semaglutide and period changes: what side-effect reports show
A 2026 analysis of a US database of suspected side-effect reports in all women aged 12 to 55 found heavy bleeding, bleeding between periods and infrequent periods reported more often than expected with semaglutide52. For tirzepatide it found bleeding between periods and clots52. It counted reports; it did not test whether the medicines caused them52.
If cycles return, pregnancy becomes possible: the NHS (UK) says anyone who could get pregnant needs contraception while using semaglutide20. If your bleeding changes, tell your doctor. In India, you can also report a suspected side effect to PvPI, the national programme, on 1800 180 3024 on working days53,54.
GLP-1 medicines, pregnancy and fertility: the rule for each medicine
The US Wegovy label says semaglutide (Wegovy, Poviztra and the generics) is stopped at least 2 months before a planned pregnancy, because it stays in the body for a long time4. NHS (UK) guidance agrees20. The US label adds that weight loss offers no benefit in pregnancy and may harm the fetus4. Wegovy used for weight is stopped in pregnancy4. The NHS says semaglutide is not used while breastfeeding20.
For tirzepatide (Mounjaro and Yurpeak), the sources differ. The US Zepbound label says it is stopped when a pregnancy is recognised; the NHS says to stop at least 4 weeks before trying5,29. Our page on who should not take semaglutide sets out both. The NHS says tirzepatide is not used while breastfeeding29.
Tirzepatide slows stomach emptying, so the US Mounjaro label says it may make the pill work less well6. This delay is largest after the first dose6. The label advises a non-oral method, or an added barrier method for 4 weeks after the start and after each dose increase6. The NHS (UK) gives the same barrier advice29. Non-oral hormonal methods should be unaffected, the label says6. For semaglutide, the US Wegovy label says trials at 1 mg a week found no effect on the absorption of oral medicines, and still asks for monitoring4.
In the Chinese semaglutide trial, 35 % of the semaglutide group and 15 % of the metformin group became pregnant naturally in weeks 16 to 4015. By then semaglutide had stopped, and all took metformin alone15. In the Slovak pilot of 20 women, 60 % became pregnant during follow-up16. The Slovak women also took metformin, and neither semaglutide nor tirzepatide is approved as a fertility treatment4,5,16. If a pregnancy happens on treatment, tell your doctor; the NHS says neither medicine is used in pregnancy20,29.
Who can be assessed for GLP-1 treatment in India if they have PCOS?
The Indian weight indications start at a BMI of 30, or 27 with a weight-related condition1,9,55. In June 2026, CDSCO's expert committee kept the cut-offs in tirzepatide's indication, because obesity is defined differently for US and Asian populations56.
The US labels rule these medicines out for anyone with MEN 2 (multiple endocrine neoplasia type 2, a genetic condition) or a personal or family history of medullary thyroid cancer (MTC)4,5,57. MTC is a cancer of the thyroid's C cells58. The NHS says they may not suit anyone who has had pancreatitis20,29. They are also stopped in pregnancy and before a planned one, and not used while breastfeeding4,5,20,29.
PharmEasy lists Wegovy, Mounjaro and every semaglutide brand as Schedule H medicines in India, sold only on prescription3,59,60. A CDSCO committee recommended in 2023 that tirzepatide be prescribed by endocrinologists or internal medicine specialists only; its national scope is not confirmed61. A health-ministry release of March 2026 names the same specialists, and cardiologists for some uses37. The government advises you to consult a qualified specialist before use35.
A doctor reviews your history, BMI, other conditions, medicines and pregnancy plans, then decides whether a GLP-1 medicine fits the indication. Talk to a Zelta specialist to start that assessment.
Frequently asked questions
What options are available if an unplanned pregnancy occurs? Tell your doctor. The NHS says to report a pregnancy on semaglutide or tirzepatide, and that neither is used in pregnancy20,29. The US Wegovy label also names a US pregnancy registry4.
Do I need contraception on Mounjaro or semaglutide? If you could get pregnant, the NHS says you need contraception on semaglutide20. On Mounjaro, the US label says the pill may work less well6. It advises a non-oral method or an added barrier method for 4 weeks after the start and each dose increase6.
