The trials show that much of the lost weight came back after people stopped, on average. In the STEP 1 extension, people had regained two-thirds of their loss a year after stopping semaglutide 2.4 mg and its lifestyle programme1,2. In SURMOUNT-4, weight rose by 14.0 % in the year after the switch from tirzepatide (Mounjaro) to placebo, a dummy injection3. These are group averages, not a forecast for you. The UK's National Health Service (NHS) says to talk to a doctor before stopping either medicine, and not to stop suddenly4,5. PharmEasy lists both as Schedule H, sold only on prescription in India6,7.
Trial averages, not a forecast. STEP 1 and its extension were funded by Novo Nordisk; SURMOUNT-4's authors included Eli Lilly employees2,3,8.
| Trial | Medicine and dose | Time on the medicine | What happened next | The number (average) | Who ran it |
|---|---|---|---|---|---|
| STEP 1 extension (semaglutide) | Semaglutide 2.4 mg a week, plus a lifestyle programme | 68 weeks; the 327 people followed had lost 17.3 % | Medicine and programme stopped at week 68; followed to week 120 | Regained 11.6 percentage points, about two-thirds of the loss; net loss 5.6 % from the start | Funded by Novo Nordisk1,2,8 |
| SURMOUNT-4 (tirzepatide) | Tirzepatide 10 or 15 mg a week, the highest dose each person tolerated | 36-week lead-in; average loss 20.9 %; 670 people then split by chance | Half switched to placebo for 52 weeks, half stayed on | Placebo group +14.0 % from week 36 to 88; tirzepatide group −5.5 %. From the start to week 88: −9.9 % (stopped) against −25.3 % (continued) | Eli Lilly employees among the authors3 |

How this page was checked. This page rests on thirty-six sources: trial abstracts and one full trial paper (global trials), the US labels for Wegovy, Zepbound and Mounjaro and the Wegovy and Zepbound Medication Guides, one US and one Indian expert advisory, NHS guides and a leaflet (UK), minutes of CDSCO (India's drug regulator), and Indian news reports, filings, releases, price tables and pharmacy listings. Every number carries a numbered source. Facts checked on 29 October 2026. A Zelta doctor has not yet reviewed this page.
Will I regain weight after stopping GLP-1?
On average, yes, and on average people in both trials were still below their starting weight a year after stopping1,3. In the STEP 1 extension, the net loss from the start was 5.6 % at week 120, down from 17.3 % at week 681. In SURMOUNT-4, the placebo group was 9.9 % below the start at week 88, against 25.3 % for those who continued3. In the STEP 1 extension, most heart and metabolic measures that had improved moved back towards their starting values1.
Semaglutide leaves the body slowly. Under the US Wegovy label, semaglutide stays in the circulation for about 5 to 7 weeks after the last 2.4 mg dose, with a half-life of about 1 week9. Under the US Mounjaro label, tirzepatide's half-life is about 5 days10. A half-life is the time the body takes to clear half of a dose. A UK NHS leaflet says these medicines slow stomach emptying, so you may feel full sooner and for longer11. In India, CDSCO records Wegovy as approved since 20 April 2022 "for chronic weight management in adults"12.
Do you have to be on Ozempic forever?
In India, Ozempic is sold for type 2 diabetes in 0.25, 0.5 and 1 mg; the 2.4 mg weight-loss dose is Wegovy, which Emcure also sells as Poviztra13,14. No trial read for this page sets a stop date. The approvals describe long-term use: like Wegovy in CDSCO's record, Sun Pharma's Noveltreat is indicated "for chronic weight management in adults"12,15. Generic brands have been sold since March 2026; the semaglutide india hub lists them16. The US Zepbound label indicates tirzepatide to reduce excess weight and maintain the reduction long term17.
How long do I need to take Mounjaro?
The sources give no fixed length. PTI reported that Mounjaro was launched in India for type 2 diabetes and for weight management in adults at a BMI of 30 or more, or 27 or more with a weight-related condition18. In June 2026, CDSCO's expert committee said removing the BMI cut-off was not appropriate, because the definition of obesity differs between US and Asian populations19. In SURMOUNT-4, the people who stayed on after week 36 lost a further 5.5 % by week 88, on average3. The mounjaro india page covers the pens, strengths and prices.
What stopping means for the Indian pens
The Mounjaro and Wegovy pens sold in India hold more than one dose, so a pause can fall mid-pen20,21. The table lists them in launch order.
Pens sold in India as reported on the dates in the sources. Storage limits are from the US labels; "leaflet governs" means the sources give no figure and the Indian pack leaflet decides9,10,20,21.

What happens if you stop Wegovy for a month and then restart it?
Under the US Wegovy label, after 2 or more missed weekly doses the prescriber restarts the dose step-up at a lower dose9. The US Medication Guide tells patients who miss 2 or more weeks to contact their prescriber about how to restart9. The semaglutide dose schedule page has the missed-dose table. In India, the pack leaflet and your prescriber govern; the Indian leaflets were not online for this page, so the rule here is the US label's. A month off also means an opened Wegovy pen may have passed the US label's 56-day limit, and PharmEasy says the Indian box states the limit9,23.

The US Wegovy label gives the prescriber two maintenance doses, 2.4 mg (recommended) and 1.7 mg, and tells them to weigh treatment response and tolerability9. Which dose, if any, is the doctor's call.
What is a weight loss plateau on GLP-1?
A plateau is a stretch when the scale stops moving; in the trials, the average loss slowed over time. STEP 1 ran 68 weeks and ended at −14.9 % with semaglutide 2.4 mg against −2.4 % on placebo2. SURMOUNT-1 (tirzepatide) ran 72 weeks and ended at −15.0 % to −20.9 % by dose32. In SURMOUNT-4, the people who stayed on had lost 20.9 % by week 36 and a further 5.5 % by week 883. The average kept falling, but far more slowly, and a plateau on your scale may sit inside that slowing.
Why does semaglutide stop working?
The trials do not show the medicine stopping; they show an average that slows, and a spread around it. In STEP 1, 50.5 % of people on semaglutide 2.4 mg lost 15 % or more at 68 weeks2. In all, 86.4 % lost at least 5 %; everyone else lost less than 5 %2. Two things a doctor may check are set out in the US label. First, the dose step: the US Wegovy schedule reaches its maintenance dose in week 179. The US label allows up to 7.2 mg for adults who have tolerated 2.4 mg for at least 4 weeks, when more weight reduction is clinically indicated9. The Indian Wegovy strengths stop at 2.4 mg12. Second, missed doses, under the US label's 2-day rule9. The semaglutide dose schedule page sets out the steps with the Indian pens.
Does Ozempic need to be weaned off?
Ozempic is semaglutide, sold in India for type 2 diabetes13. The NHS (UK) says to talk to a doctor before stopping semaglutide or tirzepatide, and not to stop suddenly4,5. For people with type 2 diabetes, it says stopping semaglutide could affect blood sugar4. Neither withdrawal trial tested a gradual step-down against a sudden stop1,3. SURMOUNT-4 compared continuing tirzepatide with a switch to placebo; in the STEP 1 extension, all treatment stopped at week 681,3. So this page cannot say that tapering changes regain. A doctor decides when and how to taper.
Some situations call for a stop. For a planned pregnancy, plan the stop with your doctor: the US Wegovy label and the NHS say semaglutide at least 2 months before trying4,9. The NHS says tirzepatide at least 4 weeks before; the US Zepbound label says to stop it when a pregnancy is recognised5,17. Severe stomach pain that does not go away, with or without vomiting: both US Medication Guides say to stop the medicine and call the prescriber right away9,17. If side effects bother you or do not go away, the NHS semaglutide guide says to speak to a pharmacist or doctor and keep taking the medicine unless told to stop4. In India, you can report a suspected side effect to PvPI, the national pharmacovigilance programme, on 1800 180 3024 on working days33,34.
If you are thinking about stopping, or your weight has stopped moving, talk to your prescriber, or Talk to a Zelta specialist, before you change anything.
How to maintain weight loss after semaglutide: what the labels pair the medicine with
On average, regain after stopping is the usual result in the trials, and this page cannot promise otherwise1,3. The US Zepbound label approves tirzepatide together with a reduced-calorie diet and more physical activity17. Sun Pharma's Indian wording for Noveltreat and Novo Nordisk India's for Wegovy say the same14,15. In STEP 1, both groups had a lifestyle programme, and the extension stopped it with the medicine1,2. A joint advisory from four US medical and nutrition societies says muscle and bone should be preserved during GLP-1 use through resistance training and an appropriate diet35. It calls for nutrition and lifestyle counselling before, during and after weight loss35. It says protein targets of 1.2 to 1.6 g per kg of body weight a day have been proposed during active weight reduction35. An Indian consensus panel adds hydration: more than 2 litres a day of water or non-sugary drinks36. Anyone with a fluid or protein limit from a doctor follows that limit.
Frequently asked questions
Will I gain weight if I stop taking Ozempic? In India, Ozempic is a type 2 diabetes medicine; the weight-loss dose is Wegovy13. When that dose and its lifestyle programme stopped in the STEP 1 extension, two-thirds of the loss came back within a year, on average1.
What happens if you stop semaglutide? On average, weight came back over the following year1. The NHS (UK) says to talk to a doctor first and not to stop suddenly4.
How long does semaglutide stay in your system? About 5 to 7 weeks after the last 2.4 mg dose, under the US Wegovy label; tirzepatide's half-life is about 5 days under the US Mounjaro label9,10.
Does Ozempic need to be weaned off? Ozempic is a type 2 diabetes medicine in India13. A doctor decides when and how to taper. The NHS (UK) says not to stop suddenly, and the withdrawal trials did not test a taper1,3,4.
What happens if you stop Wegovy for a month and then restart it? Under the US Wegovy label, the step-up restarts at a lower dose, and the Medication Guide says to contact the prescriber9. The Indian pack leaflet governs here.
Do you have to be on Ozempic forever? In India, Novo Nordisk sells semaglutide's weight dose as Wegovy, not Ozempic13. The trials cited set no stop date. CDSCO records Wegovy for chronic weight management in adults12.
Is a weight loss plateau the same as the medicine not working? The trial data show a spread, not a stop. In STEP 1, 86.4 % on semaglutide 2.4 mg lost at least 5 % and 50.5 % lost 15 % or more2. In SURMOUNT-4, those who stayed on after week 36 still lost a further 5.5 % by week 88, on average3.
